Checklist

Items to Include in Your Proposal:

____ Cover page. Use attached form with appropriate information.

____ The Plan. Six proposal components need to be included. Please number and label each component as follows:

_____1. GOAL(S). Statement(s) of what you want students to be able to do.

_____2. GROUP(S). Students that will benefit from this Plan.

_____3. OBJECTIVES. Specifically stated student behaviors in measurable terms

_____4. ACTIVITIES which will support your objective(s).

_____5. ASSESSMENTS AND MEASUREMENTS: Your Plan should be quantifiable and measurable. All participants should be accountable for assessing and measuring their participation, in addition to the measurement of benefits to students. Site developed assessments need to be submitted with the initial proposal. (This includes surveys, checklists, rubrics, questionnaires, exemplars, etc.)

_____6. TIMELINE: Set target dates and deadlines.

_____Letter of support from your community (normally, your Building Accountability Committee or Site Council).

_____Letter of support from your administrator.

  • Note: In past years, you have submitted a list of participating teachers with your proposal. To simplify the process, the list of teachers who complete the Plan will be submitted to your building principal or supervisor at the end of the Plan.
